It is our purpose to assemble, under one title, current research that addresses the varying facets that underscore the nature of sudden in-custody deaths. The intent is to provide information that can further educate and assist those officers, adm- istrators, investigators, trainers, and medical personnel who must interact, intervene, and make decisions about how to prevent sudden in-custody deaths. Sudden Deaths in Custody specifically addresses sudden in-custody deaths that occur after a violent confrontation. Such incidents may occur after police or correction officers’ intervention, but also include incidents that may occur in a mental health facility or emergency medical field setting. The deaths described in this volume all involve sudden death within minutes or hours of contact preceded by one or more of the following: violent confrontation with police or corrections personnel, forcible control measures, and behavior inf- enced by a chemical substance, or mental impairment. Incidents involving custodial suicides, homicides, accidents, fatal pursuits, or police shootings are excluded.

Expanding on the 1990 book Epilepsy and Sudden Death, edited by Lathers and Schraeder, Sudden Death in Epilepsy: Forensic and Clinical Issues reviews of the basic science of epilepsy as it relates to SUDEP. Clinical chapters study the sophisticated simultaneous ambulatory EKG and EEG telemetry and respiratory function monitoring of patients at risk for sudden death that will help identify cardiac, respiratory, and epileptogenic interactions involved. Chapters on animal models evaluate new data from studies that build on previously used models and emphasize that multiple models are needed to investigate the pathophysiology of SUDEP, to hypothesize about effective treatments, to develop pilot studies in persons with epilepsy, and to conduct confirmatory large-scale clinical trials. Contributions discuss the interaction between the central and peripheral autonomic nervous systems and the cardiopulmonary systems; proposed mechanistic factors in SUDEP; risk categories of arrhythmogenic, respiratory, and hypoxia related cardiac death; psychological, emotional, and stress related factors; the role of alcohol and drugs in seizures; and the potential mitigating properties of antiepileptic medication. Adopting a global, multidisciplinary focus to address the mystery of SUDEP, this important work provides clinicians, researchers, patients, and families with the knowledge to freely discuss the phenomenon and thereby discover the preventive treatment regimens to decrease the occurrence of SUDEP.

In 1705-1706, during the War of the Spanish Succession and two years after a devastating earthquake, an ’epidemic’ of mysterious sudden deaths terrorized Rome. In early modern society, a sudden death was perceived as a mala mors because it threatened the victim’s salvation by hindering repentance and last confession. Special masses were celebrated to implore God’s clemency and Pope Clement XI ordered his personal physician, Giovanni Maria Lancisi, to perform a series of dissections in the university anatomical theatre in order to discover the 'true causes' of the deadly events. It was the first investigation of this kind ever to take place for a condition which was not contagious. The book that Lancisi published on this topic, De subitaneis mortibus (’On Sudden Deaths’, 1707), is one of the earliest modern scientific investigations of death; it was not only an accomplished example of mechanical philosophy as applied to the life sciences in eighteenth-century Europe, but also heralded a new pathological anatomy (traditionally associated with Giambattista Morgagni). Moreover, Lancisi’s tract and the whole affair of the sudden deaths in Rome marked a significant break in the traditional attitude towards dying, introducing a more active approach that would later develop into the practice of resuscitation medicine. Sudden Death explores how a new scientific interpretation of death and a new attitude towards dying first came into being, breaking free from the Hippocratic tradition, which regarded death as the obvious limit of physician’s capacity, and leading the way to a belief in the 'conquest of death' by medicine which remains in force to this day.
