The purpose of Sudden Deaths in Custody is to provide current information that addresses the issue from a number of perspectives. It is our purpose to assemble, under one title, current research that addresses the varying facets that underscore the nature of sudden in-custody deaths. The intent is to provide information that can further educate and assist those officers, adm- istrators, investigators, trainers, and medical personnel who must interact, intervene, and make decisions about how to prevent sudden in-custody deaths. Sudden Deaths in Custody specifically addresses sudden in-custody deaths that occur after a violent confrontation. Such incidents may occur after police or correction officers’ intervention, but also include incidents that may occur in a mental health facility or emergency medical field setting. The deaths described in this volume all involve sudden death within minutes or hours of contact preceded by one or more of the following: violent confrontation with police or corrections personnel, forcible control measures, and behavior inf- enced by a chemical substance, or mental impairment. Incidents involving custodial suicides, homicides, accidents, fatal pursuits, or police shootings are excluded.

Though it is one of the most common causes of death in epilepsy patients, SUDEP is still infrequently and even reluctantly named on autopsy reports. This under-reporting equates to a lack of attention and earnest investigation into the cause, predisposition, and prevention of SUDEP. There is as yet little effort to establish an actionable strategy in the mitigation of these potentially fatal arrhythmias. Expanding on the 1990 book Epilepsy and Sudden Death, edited by Lathers and Schraeder, Sudden Death in Epilepsy: Forensic and Clinical Issues reviews of the basic science of epilepsy as it relates to SUDEP. In 1705-1706, during the War of the Spanish Succession and two years after a devastating earthquake, an ’epidemic’ of mysterious sudden deaths terrorized Rome. In early modern society, a sudden death was perceived as a mala mors because it threatened the victim’s salvation by hindering repentance and last confession. Special masses were celebrated to implore God’s clemency and Pope Clement XI ordered his personal physician, Giovanni Maria Lancisi, to perform a series of dissections in the university anatomical theatre in order to discover the 'true causes' of the deadly events. It was the first investigation of this kind ever to take place for a condition which was not contagious. The book that Lancisi published on this topic, De subitaneis mortibus (’On Sudden Deaths’, 1707), is one of the earliest modern scientific investigations of death; it was not only an accomplished example of mechanical philosophy as applied to the life sciences in eighteenth-century Europe, but also heralded a new pathological anatomy (traditionally associated with Giambattista Morgagni). Moreover, Lancisi’s tract and the whole affair of the sudden deaths in Rome marked a significant break in the traditional attitude towards dying, introducing a more active approach that would later develop into the practice of resuscitation medicine.
